 How can we help you to
Keep Safe   -   Keep Active   -   Keep doing the things that are important to 

'I have fallen or am really worried that I will fall'

What can we do to help?
We can visit to:
  1. carry out a Home Hazard Assessment
  2. give you advice on ways to reduce your risk of a fall
  3. give you information and instruction on assistive devices

'I am less active because I am scared of falling'

What can we do to help?       
We can visit to:
  1. explore how a fear of falling is restricting your ability to remain active  
  2. support and facilitate your ability to do everyday activities that are important to you
  3. find ways to help you to regain your confidence
  4. support and enable you in realistic risk taking so you can keep doing the things that make you happy  ​

 'I am in hospital and worried about falling when I get home'

What can we do to help?
We can visit you in hospital to:
  1. discuss the risks of a fall when returning home and how to reduce the risks
  2. discuss risks and ways to ensure a safe and sustainable discharge home with hospital, social work, your own care staff
  3. support you, your family and carers prior to and following discharge from hospital to make changes which will reduce your risk of a fall when discharged
